Description: Seeking a dedicated and compassionate Nurse to join our team. As a Nurse, you will play a crucial role in providing community case management support to our clients and ensuring they receive the highest level of care. You will serve as a liaison between patients, their families, and healthcare providers, addressing concerns and promoting effective communication and collaboration.

Responsibilities:
1. Medical Advocacy: Serve as a strong advocate for clients and their families.
2. Care Coordination: Assuring clients needs are addressed.
3. Health Education: Provide clients and their families with accurate and understandable health information.
4. Client Support and Assistance: Offer emotional and practical support to clients and their families, helping them navigate complex healthcare systems.
5. Ethical Decision-making: Ensuring adherence to ethical principles and standards of care.
6. Documentation and Reporting: Maintain accurate and confidential records of client interactions.

Requirements: Ability to work autonomously and exercise independent professional and clinical judgement
Foster and develop a working environment which actively promotes a high performing culture which includes a sound value of trust and respect for consumers, carers and other stakeholders
Sets and models high standards of performance and positivity for self and others
Well-developed computer skills including the use of electronic medical records systems, MS Word, Excel and web-based research
Strong leadership skills and ability to take the initiative in resolving issues/problems

Demonstrated ability to multitask and prioritize in a complex environment
Demonstrated professional/positive behavior as well as cultural sensitivity and respect
